So it's your form that has a flaw. Edit it, it's certainly that when you see the title "Description", the property is actually called something else, like "foo"!

After that, there is a well-known bug under solidworks.
When you open a basemap, one time out of three, sw displays a blank sheet.
When it does that, I close and reopen Sw

Yes bart I have the trick it's quite annoying.

This morning I start again and guess what?
