How do you extrude the material inside the round?

To remove material, you don't have to put your sketch on one side, in which case you can use the top plane (depending on the positioning).

Otherwise, just use the bottom of the round or the top and create a sketch and then make an extrusion.

I don't really understand the request

if it's to make internal material on the wall of the tube (interlocking tube type)

It's pretty simple

Creates a sketch on the thickness of the tube by converting features

and 2-way extrusion

if it's to pierce your stitching it's the same but removal of material

or plane of the middle of the perpendicular tube, sketching and removal of material
