dbz might be on the right track, several search results on the net report the same problem for SW2015 SP4.0. Either do the fix given by dbz or switch to SP5.0.

I'm also taking advantage of it because I've just cut a HUGE thorn in my side. I think that in the wake of this, the manipulation simply solved the problem of displaying all my SW icons. Too good, I can finally visualize my pieces as an icon :)
I agree, dbz's answer would have deserved more than mine!
