I'm in 2014 sp2

 

If you follow my image to the letter, it's sure to work.

 

1-Modify your sketch as on my first image.

2- remove the function, and I say about the deletion!

3-restart the function by filling in your sketch profile and your trajectory

4-Don't forget the curvature field! (see my last photo)

Fill in box number 3 Cylindrical/conical bodies and fill in the sketch line that is on your profile as in the photo I sent

@gt22 when editing the unfolded the fixed face is not defined correctly, it must be pointed at an edge of the cylindrical face at the level of the opening.

Well I'm not going to leave you alone GT22, but I can't do it in sw12sp5 either.

 

1- I don't have box 3 of Bart's image.

2- the unfolded is circular for me....

Solidworks 2012 does not support unfolded sheet metal scanning.

 

That's why the versions have evolved.

 

Otherwise there is an add-in that handles complex folds

 

LOGOPRESS, of course, is not free.
